Job summary

Wayne Memorial Community Health Centers (WMCHC) is seeking a Nurse Practitioner to join the Honesdale Family Health Center. This part-time position provides primary care to patients, working closely with physicians in the clinic under established standards and protocols.

You will obtain histories and exams, interpret data, order tests, prescribe medications within scope, and educate patients while coordinating care across pediatric and adolescent patients as needed.

Qualifications

  • Graduate of an accredited nursing program.
  • Knowledge of nursing theory, practice, and patient care.
  • Ability to work with physicians and medical staff.
  • Ability to respond calmly in emergencies.

Responsibilities

  • Obtains complete medical history and physical data on patients.
  • Interprets and integrates data to determine diagnostic and therapeutic procedures as needed.
  • Performs a complete physical exam and records findings.
  • Collaborates with physicians in managing acute and long-term medical needs of patients.
  • Orders appropriate laboratory and diagnostic procedures.
  • Administers medications and injections according to physician direction.
  • Sutures minor lacerations.
  • Interviews and advises patients regarding health and illness prevention.
  • Provides patient education.
  • Maintains and reviews patient records, charts, and other pertinent information.
  • Posts tests and examination results.
  • Triage s patient telephone calls and provides consultation.
  • Provides monitoring and continuity of care between physician visits.
  • Provides comprehensive health care to pediatric and adolescent patients as indicated.

Wayne Memorial Community Health Centers (WMCHC) is seeking a Nurse Practitioner to join the team at our Honesdale Family Health Center. This position provides primary care to patients acting as an assistant to physicians in the clinic following established standards and practices/protocols.WMCHC’s MissionWayne Memorial Community Health Centers creates and sustains healthy rural communities by providing accessible, high quality medical, dental, and behavioral health care for all, while recognizing and addressing other factors that influence health outcomes.Employment Type: Part-TimeWhat you will do as a WMCHC Nurse Practitioner:Obtains complete medical history and physical data on patients.Interprets and integrates data to determine appropriate diagnostic and therapeutic procedures as needed.Performs a complete physical exam and records findings. Collaborates with physicians in managing acute and long-term medical needs of patients.Orders appropriate laboratory and diagnostic procedures.Synthesizes data to determine preliminary diagnosis and therapeutic plan utilizing principles of prevention.Administers medications and injections according to physician direction. Sutures minor lacerations.Interviews and advises patients regarding health and illness prevention. Recommends community resources to meet patient and family needs.Instructs patient and family regarding medications and treatment instructions. Provides patient education.Maintains and reviews patient records, charts, and other pertinent information. Posts tests and examination results.Triages patient telephone calls and provides consultation.Manages medical and surgical emergencies.Provides monitoring and continuity of care between physician visits.Provides comprehensive health care to pediatric and adolescent patients as indicated.
