Wayne Memorial Hospital in Honesdale, PA is seeking a part-time, on-site Internal Medicine Advanced Practice Provider (CRNP or PA-C) to support our team.
The AP provider will conduct new patient histories and exams, manage sick visits, wound care, and post-ER/hospital follow-ups, and educate patients on diabetes and treatment plans, under collaborative guidelines. This role emphasizes team-based care and accurate EHR documentation.

We are seeking a part-time, on-site Internal Medicine Advanced Practice Provider (CRNP or PA) to support our team. The Advanced Practice Provider will work collaboratively with Internal Medicine Practitioners to provide comprehensive care and work together in a team-based model to provide care for patients with complex medical needs. The candidate will have clinical responsibilities to include but not limited to new patient history and physical intake with assessment and plan. Sick visits, wound care, transition of patient care post ER/ hospital follow-up. Patient education with focus on diabetic education and treatment plan. In basket management, EKGs, IV management as needed.
