Job summary

Wayne Memorial Community Health Centers in Honesdale, PA is seeking a Nurse Practitioner to join our Family Health Center. This role provides primary care to patients as an assistant to physicians, following established standards and protocols.

You will obtain medical histories, perform examinations, order tests, manage acute and chronic conditions, and participate in patient education and community workshops to promote health in our rural community.

Qualifications

  • Graduate of an accredited program.
  • Knowledge of nursing theory, practice, and medical care.
  • Knowledge of policies and procedures to administer patient care.
  • Knowledge of medical equipment to administer patient care.
  • Ability to maintain safe work environment and respond to emergencies.

Responsibilities

  • Obtains complete medical history and physical data on patients.
  • Interprets data to determine diagnostic and therapeutic procedures.
  • Performs complete physical exams and records findings; collaborates with physicians.
  • Orders laboratory and diagnostic procedures as needed.
  • Synthesizes data to determine preliminary diagnoses and plans.
  • Administers medications and injections; sutures minor lacerations.
  • Instructs patients on medications and treatment instructions; provides education.
  • Maintains patient records and posts test results.
  • Triages patient calls and provides consultation.
  • Manages medical and surgical emergencies.
  • Provides monitoring of care between physician visits.
  • Provides care to pediatric and adolescent patients as indicated.
  • Prescribes a limited formulary per state guidelines.
  • Provides workshops and presentations to professionals and community.
  • Develops patient education tools and participates in community programs.
  • Performs other duties as assigned.

Wayne Memorial Community Health Centers (WMCHC) is seeking a Nurse Practitioner to join the team at our Honesdale Family Health Center. This position provides primary care to patients acting as an assistant to physicians in the clinic following established standards and practices/protocols.

What you will do as a WMCHC Nurse Practitioner:
  • Obtains complete medical history and physical data on patients.
  • Interprets and integrates data to determine appropriate diagnostic and therapeutic procedures as needed.
  • Performs a complete physical exam and records findings. Collaborates with physicians in managing acute and long-term medical needs of patients.
  • Orders appropriate laboratory and diagnostic procedures.
  • Synthesizes data to determine preliminary diagnosis and therapeutic plan utilizing principles of prevention.
  • Administers medications and injections according to physician direction. Sutures minor lacerations.
  • Interviews and advises patients regarding health and illness prevention. Recommends community resources to meet patient and family needs.
  • Instructs patient and family regarding medications and treatment instructions. Provides patient education.
  • Maintains and reviews patient records, charts, and other pertinent information. Posts tests and examination results.
  • Triages patient telephone calls and provides consultation.
  • Manages medical and surgical emergencies.
  • Provides monitoring and continuity of care between physician visits.
  • Provides comprehensive health care to pediatric and adolescent patients as indicated. This may include assessment of the newborn, well child care, adolescent health care, management of acute and chronic diseases and treatment of injuries.
  • Per State Licensing guidelines, prescribe a limited formulary of medication.
  • Provide workshops, seminars and presentations to other professionals and community.
  • Develop and evaluate relevant patient education tools as appropriate.
  • As appropriate, act as director or participant of special community programs and projects.
  • Performs other duties as assigned to support efficient operation of the department.
Qualifications

Minimum Qualifications:

  • Graduate of an accredited program
  • Knowledge of professional nursing theory, practice, and medical care to give and evaluate patient care
  • Knowledge of organizational policies, regulations and procedures to administer patient care
  • Knowledge of medical equipment and instruments to administer patient care;
  • Knowledge of common safety hazards and precautions to establish a safe work environment
  • Skill in applying and modifying the principles, methods, and techniques as a professional health care provider; in taking medical history to assess medical condition and interpret findings
  • Skill in preparing and maintaining records, writing reports, and responding to correspondence
  • Skill in developing and maintaining department quality assurance, in establishing and maintaining effective working relationships with patients, medical staff, and the public
  • Ability to maintain quality control standards and ability to react calmly and effectively in emergency situations
  • Ability to interpret, adapt and apply guidelines and procedures and to communicate clearly
