Job summary

Wexford Health Sources, Inc. is seeking a PA/NP for Western New Mexico Correctional Facility North in Grants, NM.

This advanced practice role involves history and physicals, labs, diagnostics, treatment plans, and direct patient care within a correctional setting. You will prescribe medications, provide injections, wound care, and minor procedures, while counseling patients on preventive care and treatments.

Qualifications

• Graduate from an accredited NP or PA program.
• Active Nurse Practitioner or Physician Assistant license in state of practice.
• Active DEA license required.
• Active CPR certification.
• Strong diagnostic skills and ability to work independently.
• Collaborative mindset and compassionate care.

Responsibilities

• Perform history and physicals, ordering labs and diagnostics, and managing treatment plans.
• Prescribing medications and administering injections, wound care, and minor procedures.
• Counseling patients on medical issues, preventative care, and prescribed treatments.
• Coordinating patient care and specialty referrals to ensure continuity of services.
• Documenting all clinical encounters and interventions in the EMR system.
• Monitoring patient conditions and participating in daily rounds and screenings.
• Collaborating with nursing staff and security to promote safe, effective care delivery.
• Guiding and supporting nursing personnel and assisting in staff orientation when needed.

Position Summary
