Job summary

Wexford Health Sources, Inc. is seeking a Nurse Practitioner or Physician Assistant for the Western New Mexico Correctional Facility North in Grants, NM. The role involves performing exams, treating acute and chronic conditions, coordinating care, and collaborating with a multidisciplinary team.

You’ll educate, lead, and advocate for patients, with supervision from the Medical Director. A valid NP/PA license, DEA, CPR, and state registration are required.

Qualifications

• Graduate from an accredited NP or PA program.
• Active unrestricted Nurse Practitioner or Physician Assistant license in state of practice.
• Active applicable Controlled Substance Registration/License/Certification in state of practice.
• Active DEA license required.
• Active CPR certification required.

Responsibilities

• Assess and treat acute and chronic conditions as part of a multidisciplinary team.
• Perform history and physicals, order labs and diagnostics, and manage treatment plans.
• Prescribe medications, administer injections, wound care, and minor procedures.
• Counsel patients on medical issues, preventative care, and prescribed treatments.
• Coordinate patient care and referrals to ensure continuity of services.
• Document all clinical encounters in the EMR system and participate in rounds.
