Job summary

Wexford Health Sources, Inc. is seeking a Nurse Practitioner or Physician Assistant to serve as a key medical provider at South Central Regional Jail in Charleston, WV.

The role includes performing exams, managing care, and coordinating with a multidisciplinary team to deliver comprehensive, respectful care to justice-involved patients. You will educate, lead, and advocate for patients, with supervision from the Medical Director and collaboration with nursing staff and security.

Qualifications

• Graduate from an accredited NP or PA program.
• Active unrestricted NP or PA license in state of practice.
• Active DEA and CPR certifications.
• Ability to work independently and with a collaborative team.

Responsibilities

• Perform history and physicals, order labs and diagnostics, and manage treatment plans.
• Prescribe medications and administer injections, wound care, and minor procedures.
• Counsel patients on medical issues, preventative care, and prescribed treatments.
• Coordinate patient care and specialty referrals to ensure continuity of services.
• Document encounters in the EMR system and monitor patient conditions.
• Collaborate with nursing staff and security to promote safe, effective care delivery.
• Lead and support nursing personnel and assist in staff orientation when needed.

Position Summary

As a Nurse Practitioner or Physician Assistant at our facility, you’ll serve as a key medical provider within a multidisciplinary team. Under the supervision of the Medical Director, you’ll assess and treat acute and chronic conditions, perform physical exams and minor procedures, and coordinate care in collaboration with nurses, physicians, and security staff. Responsibilities include:

• Performing history and physicals, ordering labs and diagnostics, and managing treatment plans
• Prescribing medications and administering injections, wound care, and minor procedures
• Counseling patients on medical issues, preventative care, and prescribed treatments
• Coordinating patient care and specialty referrals to ensure continuity of services
• Documenting all clinical encounters and interventions in the EMR system
• Monitoring patient conditions and participating in daily rounds and screenings
• Collaborating with nursing staff and security to promote safe, effective care delivery
• Guiding and supporting nursing personnel and assisting in staff orientation when needed

What You’ll Need

• Graduate from an accredited NP or PA program.
• Active unrestricted Nurse Practitioner or Physician Assistant license in state of practice.
• Active applicable Controlled Substance Registration/License/Certification in state of practice.
• Active DEA license required.
• Active CPR certification required.
