Job summary

Wexford Health Sources, Inc. in Salem, VA, seeks a Nurse Practitioner or Physician Assistant to serve as a key medical provider within a multidisciplinary team.

Under the supervision of the Medical Director, you’ll assess and treat acute and chronic conditions, perform physical exams and minor procedures, and coordinate care with nurses, physicians, and security staff. You’ll lead patient care activities, document in EMR, participate in rounds, and guide nursing staff.

Qualifications

• Graduate from an accredited NP or PA program.
• Active unrestricted NP or PA license in state of practice.
• Active applicable CS registration/certification in state of practice.
• Active DEA license.
• Active CPR certification.
• Strong diagnostic skills and ability to work independently within your scope.
• A collaborative mindset and a compassionate, nonjudgmental approach to care.

Responsibilities

• Perform history and physicals, ordering labs and diagnostics, and managing treatment plans.
• Prescribe medications and administer injections, wound care, and minor procedures.
• Counsel patients on medical issues, preventative care, and prescribed treatments.
• Coordinate patient care and specialty referrals to ensure continuity of services.
• Document all clinical encounters and interventions in the EMR system.
• Monitor patient conditions and participate in daily rounds and screenings.
• Collaborate with nursing staff and security to promote safe, effective care delivery.
• Guide and support nursing personnel and assist in staff orientation when needed.

Responsibilities include:

• Performing history and physicals, ordering labs and diagnostics, and managing treatment plans
• Prescribing medications and administering injections, wound care, and minor procedures

• Coordinating patient care and specialty referrals to ensure continuity of services
