Job summary

Community Health Care is seeking a family-focused APN to provide comprehensive patient care in Ohio. You will perform exams, document histories, develop and adjust care plans, order tests, and diagnose health issues in collaboration with physicians.

Educating patients and families to promote prevention, while ensuring HIPAA/OSHA compliance and high-quality care in a supportive, team-based environment.

Qualifications

• Master’s degree from an accredited nursing program.
• APN authorization in Ohio to practice family health.
• Knowledge of Electronic Medical Records and information systems.
• Familiarity with HIPAA and OSHA regulations.

Responsibilities

• Performing physical exams and patient observations.
• Recording patients' medical histories and symptoms.
• Creating patient care plans and contributing to existing ones.
• Ordering and analyzing diagnostic tests.
• Diagnosing health issues.
• Detecting changes in patients' health and modifying treatment plans, as needed.
• Educating patients and their families to manage and prevent illness and injury.
• Collaborates with physicians in managing acute and long-term medical needs of the patients.

What We’re Looking For

• Knowledge of Electronic Medical Records and information systems
• A sound understanding of all pertinent federal and state regulations, including HIPAA and OSHA
• Excellent verbal and written communication skills
• Knowledge of disease processes and prevention
