The nurse acts as a liaison between patients and caregivers to ensure continuity of care through clinical support, navigation, and collaboration with providers.

Requirements include a minimum of 2 years of related experience with focus on patient education, clinical support, and effective communication.

Qualifications

• Graduate from a specialty training program (required).
• Bachelor’s Degree in Nursing (preferred).
• Skills include clinical support, patient education, collaboration, communication, and coordination.

Responsabilités

• Coordinate patient care across the care continuum.
• Provide education and navigation through treatment plans.
• Facilitate entry into multidisciplinary health programs.
• Serve as liaison between patients and caregivers.
• Support continuity of care with clinical support and provider collaboration.
• Coordinate pre- and post-hospital care; connect to support and social services.
