Job summary

LiveWell Homecare Agency is seeking an experienced RN to provide advanced nursing support within an interdisciplinary team, promoting preventative care and coordinating disease management for patients, families, and providers.

You will assess needs, develop individualized care plans, educate patients, and implement evidence-based interventions while maintaining documentation and participating in professional development to improve outcomes.

Qualifications

• RN with at least 2 years of experience.
• Graduate from Specialty Training Program-Nursing (required), Bachelor's preferred.
• Strong clinical, communication, and organizational skills.
• Ability to work collaboratively within a team in a clinical setting.

Responsibilities

• Assess healthcare, educational, and psychosocial needs; develop individualized care plans.
• Coordinate diagnostic tests and facilitate communication among patients and healthcare team.
• Educate patients and families on disease management and care coordination using age-appropriate strategies.
• Implement clinical interventions based on risk and evidence-based guidelines, collaborating with providers to adjust therapies and ensure adherence.
• Maintain documentation, manage patient interactions, and assist with triage and clinical workflows.
• Administer medications, vaccines, and IVs within scope of practice, and prepare laboratory specimens.
• Participate in professional development and post-discharge care coordination to improve patient outcomes.
