Job summary

You will act as a patient navigator, working with providers to coordinate assessments, treatment plans, and patient activities.

Qualifications

• Graduate from Specialty Training Program-Nursing (Required).
• Minimum of 2 years of LPN work experience.
• Strong communication, teamwork, and organizational skills.
• Ability to coordinate care and support patient navigation effectively.

Responsibilities

• Perform patient evaluations, education, and coordinate program activities.
• Serve as a liaison between patients, providers, and scheduling services.
• Develop and implement patient screening tools and navigational procedures.
• Investigate patient concerns related to access, communication, and billing.
• Conduct medical record reviews and assist with clinical procedures.
• Maintain clinical supplies and ensure proper operation of equipment.
