Eliot Community Human Services, Inc. is seeking a dedicated nurse case manager to support adults with mental illness in Danvers, MA.

You will coordinate medical care, deliver nursing interventions, and provide essential health education to clients in a compassionate, team-based setting. The role involves coordinating medications, updating assessments and care plans, and participating in rotating on-call coverage and evening shifts to ensure continuous support for clients.

Qualifications

• Requires an associates degree in nursing.
• Experience with adults with mental illness.
• Knowledge of psychiatric and medical medications.

Responsibilities

• Act as a primary case manager to 3-6 consumers.
• Provide nursing interventions to individuals.
• Coordinate medical care and communicate with health care providers.
• Provide health education to individuals.
• Assist in managing medications and nursing department operations.
• Update Comprehensive Assessments and IAPs for clients on your caseload.
• Transport consumers to work sites, meetings, daily activities, appointments, and activities as needed.
• Participate in rotating on-call coverage and evening shifts.
