Job summary
Eliot Community Health Services is seeking a Registered Nurse to join our compassionate team in Massachusetts. The role focuses on case management, direct nursing interventions, and coordinating medical care for adults with mental illness, while supporting health education and medication management.

Qualifications
- Requires an associates degree in nursing.
- RN nursing license.
- Experience working with adults with mental illness and knowledge of psychiatric and medical medications.
- Valid Massachusetts driver's license.
Responsibilities
- Act as a primary case manager to 3-6 consumers.
- Provide nursing interventions to individuals.
- Facilitate and coordinate medical care to include communication with health care providers.
- Provide health education to individuals.
- Assist in the management of medications and the overall functioning of the nursing department.
- Formulate and regularly update Comprehensive Assessments and IAP’s for clients on your caseload.
- Transport consumers to work sites, meetings, daily activities, appointments, and activities as needed.
- Participate in rotating on-call coverage and evening shifts to ensure 24/7 support for clients.
