Eliot Community Human Services in Boston, MA is seeking a licensed RN to join the PACT team as Nurse Manager. This role supports adults with severe mental illness through comprehensive treatment, rehabilitation, and community-based services.

The Nurse Manager will oversee nursing interventions, case management, and medication management, with outreach and in-home support as needed. Work is Monday–Friday 8am–4pm with on-call rotation; salary ranges up to $100,000.

Qualifications

• Associate degree in nursing is required.
• RN license is required.
• Experience with adults with mental illness and knowledge of psychiatric/medical medications.
• Massachusetts driver’s license is required.

Responsibilities

• Act as a primary case manager to 3-6 consumers.
• Provide nursing interventions to individuals.
• Coordinate medical care and communicate with health care providers.
• Provide health education to individuals.
• Assist in managing medications and nursing department operations.
• Formulate and update Comprehensive Assessments and IAPs for clients on your caseload.
• Transport consumers to worksites, meetings, daily activities, appointments and other activities.
• Participate in rotating on-call coverage and evening shifts for 24/7 client support.

Eliot is seeking a strong, experienced, licensed RN, Nurse Manager, to join our Program for Assertive Community Treatment (PACT) team. PACT is an evidence-based model that provides comprehensive treatment, rehabilitation, and support to adults with severe and persistent mental illness.

The ideal candidate would possess knowledge of psychiatric medications, psychiatric and medical treatment. This person would be comfortable with outreach, case management and providing support in individuals homes.

Responsibilities

• Act as a primary case manager to 3-6 consumers
• Provide nursing interventions to individuals
• Facilitate and coordinate medical care to include communication with health care providers
• Provide health education to individuals
• Assist in the management of medications and the overall functioning of the nursing department
• Formulate and regularly update Comprehensive Assessments and IAP's for clients on your caseload
• Transport consumers to work sites, meetings, daily activities, appointments, and activities as needed.
• Participate in rotating on-call coverage and evening shifts to ensure 24/7 support for clients

Qualifications

• Requires an associates degree in nursing
• RN nursing license
• Experience working with adults with mental illness and knowledge of psychiatric and medical medications
• Valid Massachusetts drivers license

Schedule

Monday-Friday 8am to 4pm with rotating 12pm-8pm shift , one weekend per month, rotating holiday shifts, rotating oncall schedule

Annual Salary: $90,000 USD - $100,000 USD
