Job summary
Eliot Community Human Services in Massachusetts is seeking a strong, experienced, licensed RN as a Nurse Case Manager to join our Program for Assertive Community Treatment (PACT) team. You will act as a primary case manager for 3-6 consumers, coordinate medical care, and provide health education and medication support in clients’ homes.
The role requires an associates degree in nursing, LPN/RN license, and experience with adults with mental illness.
Qualifications
- Requires an Associate degree in nursing.
- LPN or RN nursing license.
- Experience working with adults with mental illness and knowledge of psychiatric and medical medications.
Responsibilities
- Act as a primary case manager to 3-6 consumers.
- Provide nursing interventions to individuals.
- Facilitate and coordinate medical care to include communication with health care providers.
- Provide health education to individuals.
- Assist in the management of medications and the overall functioning of the nursing department.
- Formulate and regularly update Comprehensive Assessments and IAPs for clients on your caseload.
- Transport consumers to work sites, meetings, daily activities, appointments, and activities as needed.
- Participate in rotating on-call coverage and evening shifts to ensure 24/7 support for clients.
