Qualifications

• Specialized Diploma Graduate of an accredited school of nursing required.
• 3 years as a Registered Nurse in an acute care setting.
• Effective communication concepts and ability to engage patients and families.
• Knowledge of patient- and family-centered practice and collaboration.
• Knowledge of cultural diversity and care coordination across teams.

Responsibilities

• Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.
• Communicates with and supports interprofessional teams, patient, family and designated care partners.
• Collaborates with medical staff to ensure consistent interdisciplinary plans of care and quality patient care.
• Assesses patient needs and makes care recommendations based on physical, psychosocial, cultural needs, age and functional status.
• Facilitates timely patient throughput optimization for admissions, transfers and discharge on the unit.
• Provides schedules and on-call coverage when applicable; resolves scheduling issues.
• Monitors staff schedules to ensure adequate staffing and may authorize overtime to meet patient needs.
• Collaborates with the Nurse Manager to evaluate and improve nursing practice and performance.
