Job summary

The Providence Center is seeking a registered nurse to join the Harmony Team within the CCBHC program. The nurse will assess health needs, coordinate medical and psychiatric care, administer medications, and support recovery in both office-based and community settings.

Working with a multidisciplinary team, the nurse delivers person-centered services, maintains compliance, and promotes Wellness outcomes while traveling to community sites as needed.

Qualifications

• RN with psychiatric/mental health experience preferred.
• RI licensure required for Nursing practice.
• Valid driver's license and insurance required.

Responsibilities

• Perform comprehensive nursing assessments and update care plans.
• Provide in-office and community-based services to caseload; ensure program standards.
• Triage, assess crisis risk, and coordinate with the care team and prescribers.
• Administer medications with EMR documentation and six rights.
• Educate clients and families on diagnoses, care plans, and medications.
• Document thoroughly and maintain regulatory compliance.
• Attend team meetings and participate in trainings and orientations.
