Job summary

Care New England Providence Center is seeking a Nurse (RN or LPN) to assess health needs, coordinate care, administer medications, and support wellness in office-based and community settings. RN licensure is required for CCBHC roles, with LPN options available in some programs.

The nurse will work with a multidisciplinary team, ensure documentation accuracy, and strive for recovery-focused outcomes while following safety and infection prevention standards.

Qualifications

• Associate’s Degree or Diploma in Nursing required; RN licensure required for CCBHC roles.
• Experience in psychiatric/mental health or substance use treatment preferred.
• Rhode Island RN licensure required; LPN eligible for specific opportunities.

Responsibilities

• Perform nursing assessments and update care plans to improve client health.
• Provide in-person and community-based services to a caseload meeting program standards.
• Triage clients and assess health, psychiatric, substance use, housing, and safety needs.
• Administer medications and ensure accurate EMR documentation.
• Educate clients and families on diagnoses, care plans, and medications.
