Job summary

Care New England is seeking a Registered Nurse for the Providence Center ACT Trinity Team (CCBHC) to assess health needs, coordinate medical and psychiatric care, administer medications, and support wellness. You will work in office-based and community-based settings as part of a multidisciplinary team, delivering person-centered services and maintaining high standards of care.

Qualifications

• Associate degree in nursing required; bachelor's preferred or diploma acceptable.
• RI licensure as a Registered Nurse required.
• Experience with psychiatric/mental health patients and substance use treatment preferred.
• Ability to travel within the community with reliable transportation.

Responsibilities

• Perform nursing assessments (initial, annual, ongoing) and update care plans.
• Manage caseload with in-person contacts in office and community settings.
• Triage clients; assess health, psychiatric, substance use, housing, and safety needs.
• Develop and coordinate individualized care plans with client input.
• Coordinate hospital and post‑discharge follow‑ups; communicate changes to the care team.
