Job summary

The Providence Center Nurse ACT I Harmony Team (CCBHC) plays a vital role in assessing health needs, coordinating medical and psychiatric care, administering medications, and supporting overall wellness for clients.

Working collaboratively with a multidisciplinary team, the Nurse ensures the delivery of high-quality, person-centered services that promote recovery, stability, and improved health outcomes in both office-based and community-based settings.

Qualifications

• Requires Associate's Degree in Nursing; RN licensure in RI.
• Experience with psychiatric/mental health and substance use preferred.
• Bilingual skills preferred; driving required.

Responsibilities

• Perform nursing assessments and update care plans.
• Provide in‑person and community‑based nursing services.
• Triage, assess crisis needs, and respond to emergencies.
• Coordinate care transitions and follow‑up with prescribers.
• Document results, labs, and medications in EMR.
