Responsibilities

• Coordinate care management from entry into the healthcare system to post-discharge.
• Initiate and monitor patient care plans with the care team.
• Lead interdisciplinary team to ensure safe, cost-effective care.
• Review observation admissions for appropriateness of stay.
• Interview patients and families to identify needs and plan for discharge.
• Advocate for patients and families.
• Communicate with third-party payors for pre-certification and approvals.
• Identify delays and work with team to minimize delays.
• Establish measurable goals with case management staff and physicians.
