Job summary

Qualifications

• Associate's Degree in Nursing from a state-accredited school.
• RN license in Georgia (Required).
• 3+ years of acute care experience. (Required)
• CCM certification (Preferred).

Responsibilities

• Perform utilization review and discharge/transition planning per caseload.
• Coordinate care across acute and post-acute settings with nursing, social work and physicians.
• Educate patients and families about discharge options and services.
• Identify barriers to timely discharge and facilitate appropriate actions.
