Job summary

Socket.dev is seeking a Care Manager - RN to coordinate discharge planning, transitions of care, and case management to ensure optimal patient outcomes. You will review medical records for appropriateness and compliance with standards and collaborate with interdisciplinary teams.

This role requires an Associate Degree in Nursing, 2–4 years of clinical nursing experience, and demonstrated ability in care management, documentation, and communication.

Qualifications

• Associate Degree in Nursing required.
• 2-4 years of clinical nursing experience in hospital, home health, or nursing home setting.
• 2-4 years of care management experience preferred.

Responsibilities

• Conduct daily reviews of medical records to assess admission and utilization of services.
• Collaborate with IDT to coordinate patient care and transitions.
• Develop and implement discharge plans and post-hospital care options.
• Refer cases to physicians/managers when criteria are not met.
• Maintain accurate documentation of case management activities.
• Coordinate with community agencies to facilitate seamless transitions.
• Lead interdisciplinary meetings to address patient care needs.
