Qualifications

• Associate's Degree in Nursing and active license in the state of employment is preferred.
• BSN preferred and must be obtained within 5 years.
• 4–6 years of nursing experience preferred with demonstrated skills.

Responsibilities

• Manages an assigned caseload from preadmission to discharge.
• Coordinates communication with third party payers and Utilization Committee.
• Leads discharge planning multidisciplinary team meetings and documents outcomes.
• Assesses level of care, medical necessity, and monitors length of stay.
• Coordinates and collaborates with physicians and health care professionals.
• Revise and adjust the plan of care daily based on patient condition.
• Ensure continuity of care and appropriate use of resources.
• Participate in denials management to minimize third-party denials.
