• Contributes to cost effectiveness and efficiency; analyzes annual financial goals for the targeted population.
• Collaborates with physicians, payors, and health team to ensure appropriate discharge planning and care levels.
• Assesses patient needs for referrals and alternate levels of care; coordinates coordination with the care team.
• Identifies opportunities to prevent denials and optimize reimbursement; communicates issues to relevant staff.
• Maintains safety, quality assurance, and infection control standards; stays current with field trends.
