Job summary

Socket.dev is seeking a Professional Nurse Leader to assist the Director of Case Management in coordinating care across the full continuum, from pre-acute to post-discharge, with physicians and interdisciplinary teams.

The Case Manager will coordinate care, advocate for patients, negotiate with payors, manage transitions to long-term care, and ensure timely, cost-conscious service delivery. BSN preferred and GA license required, with 3+ years of clinical nursing experience.

Qualifications

• BSN preferred.
• Current Georgia nursing license.
• Experience in Utilization Management and Discharge planning desirable.
• At least 3 years of clinical nursing experience.
• Strong communication and teaching skills.

Responsibilities

• Coordinate care management from patient entry into the healthcare system to post-discharge.
• Initiate and monitor patient care plans with the patient care team.
• Coordinate care and resources with physicians and other team members to achieve optimal patient outcomes.
• Review observation admissions to determine appropriateness of stay.
• Interview patients and families to identify needs and arrange for medical, psychological, financial, environmental, and discharge planning.
• Advocate for patients and families.
• Communicate with payors to obtain pre-certification and approved inpatient days.
• Communicate utilization assessments to justify level of care.
• Assess level of care and time frames for appropriate services.
• Maintain a roster of community resources and make available to patients/families.
• Ensure smooth transitions to long-term care and send paperwork to facilities.
• Maintain professional relationship with medical staff.
• Identify delays in service and take steps to eliminate them.
• Work with case management team to set measurable goals for cost-effective, high quality outcomes.
