Job summary

Your Health Organization in Brunswick, GA is seeking a Registered Nurse to serve patients in the Brunswick area as a Nurse Case Manager. You will travel to patients’ homes and facilities to evaluate care and coordinate telemedicine visits with providers.

The role requires reliable transportation and a full-time schedule (8am–5pm, Monday–Friday). Previous case management and community health experience are preferred.

Qualifications

• Must be licensed as a Registered Nurse.
• At least 3 years of clinical experience preferred.
• Experience in community settings preferred.
• Strong written and verbal communication.

Responsibilities

• Facilitate receiving all medical records from providers and specialists.
• Review medical records and complete consents.
• Enroll patients in Care Management and initiate care plans.
• Coordinate care and communicate with patients and caregivers.
• Evaluate home health, RPM, or DME options as applicable.
• Document Care Management activities accurately.
• On call may be required.
