Job summary

Your Health Organization in Georgia is seeking a Registered Nurse to join our Visiting Nurse program as a Nurse Case Manager. The nurse will visit patients at home or in facilities, coordinate with providers, and initiate telemedicine visits to support quality care.

This is a full-time, salary-based position with 8‑hour shifts, Monday to Friday (8am–5pm), covering Bogart, Loganville, Winder and surrounding areas. Reliable transportation is required as travel is daily.

Qualifications

• Must be a licensed nurse.
• Minimum of three years clinical experience preferred.
• Experience in community settings preferred.
• Proven ability to communicate and collaborate with care teams, patients, and caregivers.
• Must be able to travel daily and work flexible hours.

Responsibilities

• Facilitate receiving medical records from primary providers and specialists.
• Review medical records and complete consents with patients.
• Enroll patients in Care Management and initiate a Plan of Care.
• Coordinate and communicate care with patients, families, and care teams.
• Reconcile medications and evaluate needs for home health or hospice when appropriate.
• Document Care Management activities accurately and timely.
• On-call duties may be required.
