Job summary

Your Health Organization in the Augusta area is seeking a Registered Nurse for a Visiting Nurse (Nurse Case Manager) role. You will travel to patients’ homes and long-term care facilities to evaluate care, initiate telemedicine visits, and coordinate with providers to ensure quality outcomes.

The position requires valid RN licensure, strong communication, and the ability to manage care plans while traveling daily. A full-time schedule with 8-hour shifts is offered along with competitive benefits.

Qualifications

• Must be a licensed nurse with an active license in good standing.
• Minimum three years of clinical experience preferred.
• Experience in community settings and interdisciplinary care teams desirable.

Responsibilities

• Travel to patients’ homes and facilities to evaluate care and initiate telemedicine visits.
• Facilitate receiving medical records and review them for care planning.
• Enroll patients in Care Management and initiate Plan of Care when eligible.
• Coordinate with providers, document Care Management activities accurately.
• Reconcile medications and assess safety and home environment.
• Complete AWV’s and cognitive impairment screenings; assess SDoH.
• On-call may be required.
