Job summary

Your Health Organization is seeking a Registered Nurse to join our visiting nurse care team. The Nurse Case Manager will visit patients at home and in care facilities, coordinate with providers, and initiate telemedicine visits to support high-quality, compliant care.

Travel is required daily to Bogart, Loganville, Winder and surrounding areas. This full-time, salary-based role runs Monday–Friday, 8 a.m. to 5 p.m., with opportunities for growth and a comprehensive benefits package.

Qualifications

• Licensed nurse in good standing with board.
• Three years clinical experience preferred.
• Experience in community settings preferred.
• Proven ability to communicate and collaborate with care teams.

Responsibilities

• Facilitate receiving medical records from providers and specialists.
• Review medical records and complete consents.
• Enroll patients in Care Management if eligible.
• Initiate and manage Care Plans of Care.
• Capture diagnoses with specificity and document Care Management activities.
• Coordinate and communicate care with providers, patients, and families.
• Evaluate for home health, RPM, and additional services as needed.
• Travel to patient homes and facilities; on-call may be required.
