Job summary

Your Health Organization is seeking a Registered Nurse to join our primary care team as a Nurse Case Manager. The role requires visiting patients in their homes and facilities, coordinating with providers, and initiating telemedicine visits as needed.

A reliable vehicle and daily travel are essential. The position is full-time, with 8-hour shifts from 8am to 5pm, Monday through Friday, serving Bogart, Loganville, Winder, and surrounding areas.

Qualifications

• Must be licensed as a registered nurse with a clean standing.
• Minimum of three years of clinical experience preferred.
• Experience in community settings preferred.
• Proven ability to communicate and collaborate with care teams, patients, and caregivers.
• Strong written and verbal communication skills; basic computer knowledge.

Responsibilities

• Facilitate receiving medical records from the patient’s primary provider and specialists.
• Review medical records and complete consents with patients.
• Enroll patients in Care Management if eligible and initiate a Plan of Care.
• Capture diagnoses with specificity, create gaps, and ensure acceptance.
• Complete AWVs for provider review and cognitive impairment screenings.
• Complete Social Determinants of Health assessments and ACPs for provider review.
• Evaluate for home health, hospice, palliative needs, or RPM resources.
• Coordinate and document Care Management activities with the care team.
• Reconcile prescribed and OTC medications and evaluate equipment needs.
• Evaluate for safe home environment and follow up on acute conditions.
• On-call may be required; travel daily to offices and patient locations.
