Job summary

Your Health is seeking a Registered Nurse to service patients throughout the Brunswick area as a Visiting Nurse/Case Manager. The role involves traveling to patients’ homes and facilities to evaluate care needs, initiate telemedicine visits, and coordinate with providers while adhering to laws and policies.

This full-time position runs 8-hour shifts, Monday–Friday, 8am–5pm. Ideal candidates will have RN licensure (RN preferred), strong communication skills, and at least three years of clinical

Qualifications

• Must be a licensed nurse (RN preferred).
• Minimum of three years clinical experience preferred.
• Experience in community settings preferred.
• Proven ability to effectively communicate and collaborate with interdisciplinary care teams, patients, and caregivers.

Responsibilities

• Facilitate receiving all medical records from the patient’s primary provider and specialists.
• Enroll patients in Care Management if they meet eligibility criteria.
• Initiate a Care Management Plan of Care if the patient is eligible.
• Coordinate care, communicate with the care team and providers.
• Reconcile prescribed and OTC medications and provide post-discharge education.
• On call may be required.
