Job summary

Your Health Organization seeks a Registered Nurse to join our primary care team as a Nurse Case Manager. The nurse will visit patients in their homes, coordinate with providers, and initiate telemedicine visits to ensure timely, quality care.

This full-time role requires reliable transportation and daily travel to service areas including Bogart, Loganville, Winder and surrounding areas. Shifts are 8 am–5 pm, Monday–Friday, with on-call potential and ongoing care management responsibilities.

Qualifications

Must be a licensed nurse (RN preferred) with a valid license in good standing.
Minimum of 3 years clinical experience preferred.
Experience in community settings preferred.
Strong written and verbal communication and collaboration with care teams.
Basic computer knowledge and documentation skills.

Responsibilities

Facilitate receiving medical records from the patient’s primary provider and specialists.
Review medical records and complete consents.
Enroll patients in Care Management if they meet eligibility criteria.
Initiate a Care Management Plan of Care if the patient is eligible.
Capture all diagnoses with highest specificity and reconcile medications.
Complete AWV’s and cognitive impairment screenings.
Complete SDoH assessments and/or screenings.
Evaluate for home health, hospice, palliative, or RPM devices.
Communicate and coordinate care.
On call may be required.
