Team Select Home Care is seeking a Case Manager (RN) in St. Louis, MO to oversee and coordinate care for assigned patients under physician orders and the plan of care.

You'll interact with families, coordinate services, and ensure documentation and regulatory requirements are met across the patient’s care team. The role requires valid RN licensure, at least one year of nursing experience (home health preferred), plus strong verbal and written communication skills.

Formación

• Graduate of an approved school of professional nursing and currently licensed in the state(s) in which practicing.
• Minimum (1) year nursing experience required; prior home health experience preferred.

Responsabilidades

• Develops, delivers, revises, and evaluates individualized patient-centered care plans under physician supervision.
• Provides case management to an assigned group of patients and ancillary staff.
• Counsels patient and family and educates on home health strategies and procedures.
