Responsibilities

• Perform care management for members with complex and chronic needs.
• Assess, develop, implement, coordinate, monitor and evaluate care plans.
• Provide telephonic or on-site care coordination, including discharge planning.
• Coordinate internal and external resources to meet identified needs.
• Interface with Medical Directors and Physician Advisors on care plans.
