Zusammenfassung

Your Health Organization in Myrtle Beach is seeking a Registered Nurse to join our growing primary care team as a Nurse Case Manager. You will visit patients at homes and facilities within the Carolina Forest, Myrtle Beach, North Myrtle Beach, and Loris areas, coordinating with providers and telemedicine when needed.

The role requires reliable transportation for daily travel and a full-time salary-based position with standard Monday–Friday 8am–5pm shifts.

Qualifikationen

Must be a licensed nurse (RN) and in good standing.
Minimum of 3 years clinical experience preferred.
Experience in community settings preferred.

Aufgaben

Facilitate receiving medical records from patient providers.
Review medical records and complete consents.
Enroll patients in Care Management if eligible.
Initiate a Care Management Plan of Care for eligible patients.
Coordinate and document Care Management activities and care plans.
Evaluate for home health, hospice, or RPM resources as needed.
Travel daily to patient homes, facilities, and clinics to support care.
