Job summary

Mainline Health Systems, Inc. seeks a Clinical Case Manager to collaborate with the health services team, under the Clinic Manager, to enhance a comprehensive care delivery system aligned with the Patient Centered Medical Home model.

The role involves supporting quality improvement, patient communication, and care coordination across services. Prior nursing licensure and at least one year in a medical setting are required; schedule is 8:00–5:30, M-Thu, with Friday mornings, with possible

Qualifications

• Clear communication and ability to follow direction within a team.
• Detail-oriented with attention to accuracy.
• Ability to navigate and use electronic medical records (EMR).
• Minimum of 1 year experience in medical field/setting.
• Unencumbered Nursing License required.
• Ability to work with alertness and safety in a healthcare environment.

Responsibilities

• Handle clinic telephone encounters and patient communications.
• Manage clinic paperwork, refill requests, and notify lab results.
• Process patient and staff requests via electronic messages.
• Interact with patients and staff daily in a courteous, professional manner.
• Communicate with management regarding issues.
• Participate in monthly clinical staff meetings.
• Be part of the care team and support Patient Centered Medical Home workflows.
• Attend and complete continuing education as needed for MHSI.
• Participate in daily huddles and ongoing training.
