• 3+ years of Case Management experience in healthcare.
• Knowledge of discharge planning and care transitions.
• Strong communication and advocacy skills.

Responsibilities

• Design discharge pathways aligned with patient values and family preferences.
• Advocate for patient resources and remove barriers to care.
• Evaluate documentation accuracy and advise on levels of care.
• Collaborate with care transition team to continuously improve health plans.
