Mercy Medical Center, Inc. in Baltimore, MD is hiring an RN Case Manager to assess, plan, implement, coordinate and evaluate patient care in collaboration with physicians and a care team. You will oversee discharge planning and inpatient length of stay while promoting quality and cost-effective outcomes.

The role requires a Nursing degree and valid RN license, with preference for 3+ years in a clinical setting and experience in managed care and utilization review using MCG.

Qualifications

• Must possess a degree in Nursing.
• Valid RN License.
• 3 years in a clinical setting; Medical/Surgical experience preferred.
• Managed care experience preferred.
• Case management experience preferred.
• Utilization review skills using MCG preferred.

Responsibilities

• Assess, plan, implement, coordinate, monitor and evaluate options and services to meet an individual's healthcare needs, through communication and the use of available resources to promote quality, cost effective outcomes.
• Collaborate with the physician, oversee the inpatient LOS, denied days, resource utilization and discharge planning.
• Coordinate with physicians, social workers and staff; maintain detailed records and provide crisis intervention when necessary.
