Job summary

ChenMed is seeking a Registered Nurse (RN) Case Manager in Chicago, IL to oversee transitions of care and coordinate high-quality patient care across the continuum. The role emphasizes advocacy for patients and close collaboration with physicians, social workers, and care teams to ensure effective plans of care.

The RN Case Manager 1 will maintain detailed documentation, participate in care planning, and engage families to support informed decisions and optimal resource use.

Qualifications

• Strong interpersonal and communication skills.

• Ability to work autonomously is required.
• Knowledge of nursing and case management theory and practice.
• Spoken and written fluency in English.

Responsibilities

• Manage transitions of care, discharge planning, and follow-up.
• Establish trusting relationships with patients and caregivers.
• Collaborate with clinical staff to develop and execute plans of care.
• Coordinate social services and case management across pre- and post-acute settings.
• Document care plans and progress notes in the EMR.
• Communicate with PCPs and hospital teams to optimize patient outcomes.
