The CM collaborates with social work, nurses and a multidisciplinary team to optimize outcomes, aligns with utilization plans and payer requirements, and ensures timely progression along a defined plan of care under leadership

Qualifications

• 3+ years of acute care nursing experience.
• 1+ years in case management or utilization review.
• Current Georgia RN license.
• CCM certification preferred.

Responsabilités
