Phoebe Putney Health System seeks a Nurse Case Manager to coordinate patient care from entry through discharge, ensuring timely admissions, appropriate levels of care, and cost-effective service delivery. The CM conducts first-level reviews per clinical criteria and works with social work, physicians, and the care team.

Under supervision of the Care Management leadership, the CM develops discharge plans, documents activities in the electronic system, and supports quality and payer requirements

Qualifications

• Associate's Degree in Nursing is required.
• Bachelor's Degree in Nursing is preferred.
• 3+ years of acute care experience is required.
• 1+ year of case management or utilization review is preferred.
• RN license in Georgia is required.
• CCM certification is preferred.

Responsabilités

• Coordinate patient care from entry to discharge.
• Perform utilization review using clinical criteria.
• Collaborate with social work, physicians, nurses and the multidisciplinary team.
• Develop, facilitate and implement discharge plans.
• Document CM activities and maintain required records in the electronic system.
