Job summary

Phoebe Putney Health System is seeking a Nurse Case Manager to coordinate care from entry to discharge across inpatient and observation services. The CM will perform first level clinical reviews, support utilization management, and collaborate with social work, physicians, and the care team to ensure timely, appropriate, and cost-effective patient care.

Qualifications

• Associate's Degree in Nursing from a state accredited school is required.
• RN license in Georgia is required.
• 3+ years of recent acute care experience; 1+ year of CM or Utilization Review preferred.

Responsibilities

• Coordinate care across the patient’s hospital stay and discharge planning.
• Perform care management assessments and develop individualized plans of care.
• Educate physicians, nurses and staff on CM processes and discharge planning.
• Lend expertise to multidisciplinary teams and ensure regulatory compliance in utilization management.
• Document CM reviews and ensure timely communication across care teams.
