Job summary

Phoebe Putney Health System is seeking a Nurse Case Manager to coordinate patient care from entry to discharge, ensuring timely admissions, appropriate levels of care, and efficient discharge planning. The CM will work with physicians, nurses and social workers to optimize outcomes and manage resources in alignment with the hospital's utilization plan.

The role requires an RN license (Georgia) and at least 1 year of acute care experience, with preferred CM certification and higher nursing

Qualifications

• 1+ year of acute care experience
• 1+ year of case management or utilization review experience preferred
• RN with current Georgia license required
• Certified Case Manager upon hire preferred

Responsibilities

• Coordinate care across inpatient, observation and discharge planning.
• Perform prospective or concurrent reviews using established criteria.
• Educate physicians, nurses and staff on CM processes and payer/regulatory requirements.
• Collaborate with social work, physicians and multidisciplinary teams to optimize patient outcomes.
