Job summary

Qualifications

• Associate's Degree in Nursing from a state accredited school is required.
• Current Georgia RN license is required.
• 3+ years of acute care experience; 1+ year in case management or utilization review preferred.
• Certified Case Manager (CCM) preferred.

Responsibilities

• Performs utilization review and discharge planning per assigned caseload.
• Educates physicians, nurses and staff on Care Management processes and payer policies.
• Documents CM reviews, referrals, and outcomes in the electronic system.
• Assesses patient needs and coordinates care across acute and post-acute settings.
• Identifies delays or barriers to timely care and facilitates appropriate actions to prevent readmissions.
