Qualifications

• 1+ years of Case Management or Utilization Review experience in a hospital or related setting (Preferred)
• RN license in Georgia (Required)
• CCM (Preferred)

Responsibilities

• Coordinate care across acute and post-acute settings with social workers and physicians.
• Perform utilization review and discharge planning per caseload; educate staff and patients/families.
