Job summary

Phoebe Putney Health System in Sumter, GA is seeking a Nurse Case Manager to coordinate patient care from entry to discharge across PPHS facilities. The CM will address utilization of resources for efficient and effective care delivery and collaborate with social work, physicians and the interdisciplinary team to ensure quality outcomes.

The role requires an RN license in Georgia, 3+ years of acute care experience, and CCM is preferred.

Qualifications

• Associate’s Degree in Nursing from a state accredited school.
• Bachelor’s Degree in Nursing from a state accredited School of Professional Nursing (Preferred).
• 3+ years of recent acute care experience in the assigned area. (Required)
• 1+ years of Case Management or Utilization Review experience in a hospital or related setting. (Preferred)
• Registered Nurse (RN) license with current Georgia license (Required)
• Certified Case Manager (CCM) (Preferred)

Responsibilities

• Performs utilization review and discharge/transition planning for assigned caseload.
• Assesses patient needs and coordinates with Social Worker, physicians and interdisciplinary team.
• Educates physicians, nurses and staff about CM processes and payer requirements/regulations.
• Documents CM activities and ensures compliance with regulatory and hospital standards.
