Responsibilities

• Facilitates communications among patient/family and multidisciplinary teams to resolve barriers to care.
• Collaborates with care teams to ensure quality, efficient, and cost-effective healthcare services.
• Uses evidence-based approaches to increase patient activation and engagement in their own care.
• Identifies barriers to learning and delivery of care and develops plans with the team.
• Manages CM caseload per department expectations and ensures timely documentation for regulatory requirements.
• Coordinates patient information and transitions of care between facilities and care partners.
