Bluestone Physician Services seeks a Nurse Practitioner or Physician Assistant to provide on-site, team-based primary care in Assisted Living and Memory Care communities around Blaine, MN.

Qualifications

• Graduate of an accredited Nurse Practitioner or Physician Assistant Education Program
• Board Certified/Board Prepared through AANP, ANCC, or NCCPA
• Licensed or ability to obtain licensure through the Board of Nursing or Board of Medical Practice
• Ability to obtain and/or maintain DEA certification for the State of required work

Responsibilities

• Provide on-site, team-oriented primary care to patients within Assisted Living and Memory Care communities
• Full-time providers care for patients Monday-Thursday; option for Fridays to be a clinical or an administrative day
• Manage a panel of 200 patients (approx. 12-16 per day with 14 avg.)
• Ability to design and manage your schedule
• Develop and implement care plans appropriate to patients and their desires
• Clinical support from Clinical Assistants and Care Coordination through LPNs, RNs and Social Workers
• Access to diagnostics such as labs and x-rays as needed
• Collaborate with community partners within home care, hospice, and other resources as appropriate
