Job summary

Bluestone Physician Services is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) for on-site primary care in Assisted Living and Memory Care communities around Shoreview, MN. You will join a collaborative, team-based practice delivering proactive, individualized care.

Full-time schedule Monday–Thursday, with an optional Friday; manage ~200 patients; access to diagnostics and a multidisciplinary support team; competitive salary and benefits including CME, unlimited PTO, and

Qualifications

• Graduate of an accredited Nurse Practitioner or Physician Assistant Education Program.
• Board Certified/Board Prepared through AANP, ANCC, or NCCPA.
• Licensed or ability to obtain licensure through the Board of Nursing or Board of Medical Practice.
• Ability to obtain DEA certification as required by the state of work.
• Proven ability to communicate effectively with strong verbal skills.
• Excellent interpersonal and customer service skills.

Responsibilities

• Provide on-site, team-oriented primary care to patients within Assisted Living and Memory Care communities.
• Full-time providers care for patients Monday-Thursday; option for Fridays to be a clinical or an administrative day.
• Manage a panel of 200 patients (approximately 12-16 patients per/day with 14 being the avg.).
• Ability to design and manage your schedule.
• Develop and implement care plans appropriate to patients and their desires.
• Clinical support from LPNs, RNs and Social Workers.
• Access to diagnostics such as labs and x-rays as needed.
• Collaborate with community partners within home care, hospice, and other resources as appropriate.
